Templates on Forms

Every once in a while I stumble on a feature of SAM I've never seen before and gasp. This happened last month when I found the option to place a template on top of a non-public form. Why would you want to do this? I could think of a few reasons:

  • To provide clarification for your staff about a case or what a form is form

  • To show additional information when the form is printed

Add A Template to a Form:

Only available for non-public view forms

  1. Open the form in Form Customizer

  2. On the sidebar of the form in Form Customizer, click Advanced Options > Form Display Properties

  3. You’ll notice a section called Header-Footer-Templates which is where you can choose to add a template to the top of bottom.

Notes:

  • SAM will show all templates, but in order for the form and template to function, you need to make sure the template matches the entity of the form. For example, adding a template based on Family-Case to a form based on Family.

  • Choose to show the template only while printing is a handy to way to use the form as a printable asset instead of creating a template to show the same information. You could, for example, print the Adoptive Child Preference form for your team, and add a template at the top to show only when printing that includes the family’s name, address, phone number, case type, and current social worker.

  • To remove a template go back to Advanced Options > Form Display Properties and click on the template. The popup window will appear. Next to the template you’ve previously selected, there is a Set as Blank option which will remove the template.

Internal Clarification:

In this example, I used a template to provide additional information to my staff viewing this internal form. I used the Case Worker and phone number fields to auto fill the appropriate information:

Put Printed Forms to Work

SAM forms are already optimized for printing, but you may want to include more information when you print them out without adding those fields to the form itself. This is possible if you add a template.

In this example, I’ve created a very basic “Family Contact Info” template:

I’ve added it to the form and enabled the option to only show when printing. I also enabled to use the fonts and formatting of the template, instead of our site’s default styling.

Here’s the form while it’s open… nothing at the top…

But the information appears when I go to print the form, nifty!
